Perimenopause symptoms: what the transition actually looks like

It usually starts in the forties and can last years. The signs go well beyond hot flashes.

Perimenopause is the stretch before your periods stop for good. It typically begins in the mid-forties, though it can start in the late thirties, and it commonly lasts four to eight years. Menopause itself is a single point — twelve consecutive months without a period. Everything before it is perimenopause.

What changes

Your cycle. Usually first. Cycles get shorter, then longer, then skip. Flow can get heavier or lighter, and often does both at different times.

Temperature. Hot flashes and night sweats. They vary enormously — some people get a handful over a year, others several a day for years.

Sleep. Sometimes from night sweats, often not. Waking at three and not getting back down is a common description.

Mood. More anxious, more irritable, flatter, more easily moved. Not the same as depression, though the two can sit together.

Thinking. Word-finding pauses, losing the thread, forgetting why you came into a room. Often described as brain fog, and often the symptom people find most alarming.

Body. Joint aches, headaches, vaginal dryness, changed libido, palpitations.

Perimenopause is diagnosed from your history, not from a blood test — hormone levels swing so much through the transition that a single measurement rarely settles anything. What a clinician works from is the pattern: what changed, when it started, how often it happens.

  • Bleeding that is unusually heavy or lasts longer than a week
  • Bleeding between periods or after sex
  • Any bleeding after twelve months with no periods
